Both the process of partnership working between health and social care and an emphasis on outcomes of service delivery have featured prominently in recent policy initiatives in the UK. This article describes the findings of a national research project on the outcomes for service users of partnership working in Scotland, and the practical implementation of an outcomes-based toolkit developed through a subsequent collaboration between the Scottish government and the researchers. The toolkit is designed to involve users and unpaid carers in performance management of community care in Scotland.
