Working across boundaries to improve health outcomes: a case study of a housing support and outreach service for homeless people living with HIV

This paper reports the findings of an evaluation of the 'Housing Support, Outreach and Referral' service developed to support people living with HIV who were homeless or at risk of homelessness. The service was set up as part of the Supporting People Health Pilot programme established to demonstrate the links between housing support, health and social care by encouraging the development of integrated services. Two workers were employed to assist clients with setting up a housing tenancy and to provide ongoing support to ensure that the tenancy was sustained. The support workers also made sure that clients were registered with the full range of local primary, secondary and specialist health services and understood how to access them.
