Proposals include a new system for reporting adverse incidents, performance appraisal for all doctors underpinned by revalidation, checking suitability of doctors at the appointment stage, the establishment of a National Clinical Assessment Authority to advise on sorting out poor peformance by doctors, and monitoring the quality of NHS Services through the Commission for Health Improvement.

NHS managers are coming under heavy political pressure to deliver improvements in health services. They are also being used as scapegoats by a government extremely sensitive to bad publicity whenever a problem or an error is uncovered by the media.
