There are varied and multiple triggers for homelessness amongst older people. They also face a range of barriers to accessing health services. Individuals may present with multiple health problems rooted in much earlier life episodes. Article reports on discharge and referral problems, on community practitioners' role vis-à-vis primary care and social services, on primary care trust services for homeless people, and on recent developments in partnership working, including pilot project funding from the Department of Health's Older People and Disability Division.
