The toolkit enables care providers to empower individuals to discuss and plan the care that they would prefer to receive at the end of their lives, including where they would like to die. The goal is to give care providers the information and guidance they need to be adequately prepared and confident to support the advance care planning (ACP) process.

The toolkit briefly sets the scene in terms of national context and the legal framework. Benefits of ACP are highlighted and an overview of how, why and who to involve prior to and during implementation is given. Tools and additional resources that are available to support professionals are included
