The Health Foundation is working hard to lead a stepwise change in thinking about patient safety, and this event formed part of a programme of work it is undertaking to develop a framework to answer the question How safe is care today? It wants to build on a culture that has focused almost exclusively on measuring past harm and enhance this to incorporate a proactive approach to measurement that also establishes the presence of safety. At this roundtable, experts from the fields of academia, public policy, regulation and improvement came together to share their knowledge and experience and to consider a more comprehensive approach to measuring harms in healthcare
